Job Duties and Responsibilities

  • Responsibilities include Provides ongoing assessment of residential patients for significant signs and symptoms of substance withdrawal
  • Triaging as needed for medical conditions/concerns as appropriate
  • Transcribes admissions medications, or new medications, on Treatment MARS for Individual Treatment patients
  • Administers the group of medications to Treatment Clients that Medication Techs are not allowed to administer (Insulin, et. al.)
  • Documents administration of such in Treatment Clients MARs.
  • Monitors patients for side effects of any and all medications that are administered.
  • Implements patient care plans, evaluates effectiveness of interventions, alters interventions as necessary
  • Documents information pertinent to the patient’s condition and care, in the EMR
  • Oversees performance, and works in concert, with CNA’s each shift to ensure each patient is adequately monitored for early detection of any deterioration in medical condition
  • Establishes communication and trust with each residential patient, and assists them in exploring their desire to seek ongoing care
  • Collaborates with Case Managers regarding each residential patient’s appropriate level of care after completion of the residential program
  • Work in partnership with Medication Techs in regards to residential medications prescribed and assist Medication Techs in education/administration of medications.

Qualifications

  • Previous hospital, mental health or psych experience preferred
  • Ability to adapt to working in different facilities is essential
  • 12 months of LPN experience is required
  • Active LPN license
